- 1. The applicant should be a Life Member of ISVIR for at least 10 years at the time of nomination.
- 2. The applicant should have attended a minimum of ten national ISVIR events, including Annual Conferences, ISVIR-accredited CMEs and Registry meetings.
- 3. Member should not have had any disciplinary proceedings or departmental action initiated at the place of work or within ISVIR.
- 4. Member should have satisfactorily fulfilled responsibilities assigned by the Society and should not have defaulted on obligations assigned by ISVIR.
- 5. The age of the applicant should be 50 years or above.
- 6. The applicant must obtain a minimum score of 50% (150/300) to be eligible for the award.
